October 7, 2024
Dear Patients:
It is with mixed emotions that I am announcing my retirement from active
practice, effective November 11, 2024. It has been a great pleasure
providing for your health care needs over the years, and it is not easy for
me to give it up.
As of November 11, 2024, the remaining physicians at Central Texas Heart
Center will be taking over my practice. Of course, you may seek medical
care from another physician of your choice. If you choose to transfer your
care to another physician outside of Central Texas Heart Center, I
recommend looking for a new physician as soon as possible.
Your medical records are kept confidential at Central Texas Heart Center,
and a copy of your records can be transferred to another physician or
released to you or another person you designate only through your written
permission. You will be required to sign a Medical Records Release
Authorization form and return the form to our office as soon as possible so
your records can be transferred to your new physician. If you choose to
continue your care with Central Texas Heart Center, nothing is required on
your part other than to let us know which provider you wish to see.
I have greatly valued our relationship and thank you for your loyalty and
friendship over the years. Best wishes for your future health.
Sincerely,
Gordon G. Mitchell, M.D.
